I have an issuing CA that stopped issuing Certificates. The following entry
was in the application log when the CA stopped issuing Certificates
Source: ESENT
Category: Transaction Manager
Event ID: 623
Certsrv.exe (580) The version store for this instance (0) has reached its
maximum size of 20MB. It is likely that a long-running transaction is
preventing cleanup of the version store and causing it to build up in size.
Updates will be rejected until the long-running transaction has been
completely committed or rolled back.

Stopping and restarting the CA has resolved the problem, but I would like to
know what is causing this problem.
