Someone where? Someone inside? Someone outside? Using your mdaemon server as
the SMTP server, or another one? Using one of your *accounts* or merely
relaying through it?

You should be able to control who can relay mail through your server -
either disable relay or require authenticated relay, and make sure you have
good, complex passwords for all accounts. Then configure your mail clients
to authenticate to the SMTP server in the Internet mail account settings.
I don't know where you set this up in Mdaemon, but it should be in your
documentation or you should be able to find something on the web.
